Subject: Your Response Required: Important Information for Students Participating in On-Campus Employment, Internship, or Research 

Dear Seawolves,

As previously communicated, given the COVID vaccine’s FDA approval, per New York State policy, all SUNY students are now required to submit proof of their COVID vaccination.  Stony Brook’s deadline to submit proof of vaccination isFriday, September 24.  

This requirement also applies to all students who come to campus for an on-campus job, internship, or research activities.  Our records indicate that you are employed as a student assistant or research assistant and have not submitted proof of your COVID vaccine. 

As a fully online student, please complete ONE of the following two options:

Option 1:  If you are a fully online student, but do or plan to come to campus for your student employment, internship, or research, or to use an in-person campus service or facility this fall, please submit your proof of vaccination to the Wolfie Health Portalas soon as possible, but no later than Friday, September 24, 2021.  

Please refer to the following message for details on submitting proof of COVID vaccination.  If you do not submit proof of vaccination by September 24, you will not be permitted to continue with your campus employment, internship, or research assistant activities. 

Option 2:  If you are a fully online student whose student employment, internship, or research activity is fully remote and you will not use any in-person West, East, or Southampton campus service or facility this fall semester, please fill out this Online Classes Only Attestation Form if you have not already done so. 

Upon submission of your attestation, you will not be required to submit proof of your COVID vaccination. 

NOTE: Fully online, unvaccinated students identified as having been in-person on the Stony Brook campus utilizing in-person services or facilities will be referred to the Office of Student Conduct and Community Standards for violating the Code of Student Responsibility.
